Health Guarantee / Buyer Contract

This contract is made on the ____ day of ___________ between Adva Lake and _______________ for the sale of the following puppy:

**BREEDER AGREES:**  
The puppy will be up to date on vaccinations and deworming and will provide AKC registration paperwork and lifetime support.

**72-Hour Health Guarantee:**  
The puppy is guaranteed to be healthy at delivery. If the puppy is found to be unhealthy, a written veterinarian statement must be provided, and the puppy should be returned within 72 hours for a replacement.

**BUYER AGREES:**  
The buyer must have the puppy vet-checked within 72 hours (excluding Sundays and holidays). The buyer assumes care and responsibility at the time of transfer.

This contract is non-transferable.

**Congenital Defect Guarantee:**  
Valid until the puppy is one year old plus one day. The puppy must be registered with AKC and assessed by a licensed veterinarian. Some defects may qualify for limited reimbursement, up to $500, with proof of vet bills.

**Health Issues:**  
Notify us within 24 hours of any health concerns. The owner's vet must forward relevant information to Adva Lake’s vet within 48 hours. The breeder reserves the right to void the guarantee in cases of misuse or dishonesty.
